Appointment Scheduling
Our staff will make all attempts to suit your needs to schedule at a time that is most convenient for you. We ask that if for any reason, you cannot make the appointment scheduled, that you give us a 24 hour notice. Note that up to 1 hour of time is reserved for your appointment and that a missed appointment is lost productivity for the doctor and his staff and more importantly another patient could be scheduled in your allotted time period. We do have a $40 cancellation fee if a 24 hour notice is not respected.

Your First Visit
We ask you to bring written X-ray reports and/or MRIs as well as doctor’s notes if you have them in hand. Otherwise we will try to request them from your referring doctor if applicable. For chiropractic care, Dr. Res does not always need radiographic reports to perform his treatment but will refer you to nearby imaging centers if he deems necessary.

You may facilitate the paperwork by downloading and filling out the forms at your convenience at home

or arrive at the office at least 15 minutes earlier to fill out the same forms here in the office. If you would like to fax (at 772-539-8515) the information along with any other relevant information prior to your visit, we would definitely appreciate it and it would greatly facilitate the first visit.

Fees for Services Rendered
We do participate in several plans which change from year to year. We do accept Medicare but not Medicaid. We advise all patients to call their insurance carriers to see if we are in network with them and what the co-pays if any are applicable. If this is a problem for you, we will willingly make that call for you. For your information however, as the insured and payer for the insurance, you will receive a more attentive and receptive insurance person. We do accept Visa and MasterCard for services rendered and if your financial situation is difficult, we can make suitable financial arrangements.
If you are out-of network, we will provide will send the completed forms for you to your insurance company so you can be re-imbursed for your payments here at Active Lifestyles.
I forewarn all those looking for a bargain. You will definitely get a bargain here at Active Lifestyles, as you get a doctor with twice the amount of education as the compared to the average chiropractor and who will spend quality time with you. You get value for your money. Just don’t shop for the lowest price as you will be disappointed!
